York Suburban Lacrosse Club :: Registration

The registration information on this page is applicable to youth teams (U9-U15) only. For registration information for the Men's High School, Women's High School and Women's Middle School teams, contact a coach for that team.

Registration for the 2010 season is OPEN for all teams.

Follow the instructions below to register a player for the 2010 season. You have three options - attend a registration event, online registration or registration by mail.

 

Registration Events

November 12, 2009 7:30-9:00 PM @ York Suburban High School Room 121

December 1, 2009 6:30-8:30 PM @ Indian Rock Elementary School

December 3, 2009 7:00-8:00 PM @ York Suburban Middle school lobby

 

Items to bring to registration:

  1. Completed registration packet
  2. Copy of your player's birth certificate
  3. Copy of your player's insurance card
  4. Check for registration fees
  5. Proof of US Lacrosse Membership

US Lacrosse membership is required to register with the club. You must register with US Lacrosse for the season. You can obtain a US Lacrosse membership online using the link below. Visit the 'Membership Information' section and choose to 'Apply or Renew Online':

Online Registration

  1. Access the online registration form which is submitted using Home Teams Online. The link below will open in a new browser window:

    Please note: US Lacrosse membership is required to register with the club. You must register with US Lacrosse for the season. You will need to enter your US Lacrosse membership number (provided to you following your US Lacrosse membership order) when you complete the online registration form. You can obtain a US Lacrosse membership online using the link below. Visit the 'Membership Information' section and choose to 'Apply or Renew Online':

  2. A copy of the player's birth certificate needs to be mailed to the club. Please ensure a copy of the player's birth certificate is sent to the club immediatly following online registration. Birth certificates can be mailed to:

After you have submitted your registration online and have mailed in your birth certificate, your information will be processed and you will be contacted by the club to inform you everything is in good order. Until such time as your information has been processed completely, participation in club activities where there is a possibility of injury, such as practices, scrimmages and games will be limited.

Registration by Mail

  1. Download and complete the Registration Packet. The packet contains the following documents: a Registration Form, an Amateur Athletic Waiver & Release of Liability Form, a Medical Form, a Proof of Insurance Form and a Payment Form.     Please note: US Lacrosse membership is required to register with the club. You must register with US Lacrosse for the season. Return proof of payment (printed copy of your receipt) and your US Lacrosse membership number (provided to you following your US Lacrosse membership order) with your registration packet. You can obtain a US Lacrosse membership online using the link below. Visit the 'Membership Information' section and choose to 'Apply or Renew Online':

    Please note: A copy of the player's birth certificate needs to be attached to the registration packet. Please ensure a copy of the player's birth certificate is attached prior to submitting the completed registration packet.

  2. Return the complete registration packet with a check made payable to 'York Suburban Lacrosse Club' at the registration event, or mail to:

REFUND POLICY: Any cancellation requests received on or before March 1st, will receive a refund of the paid registration amount less a $25 cancellation fee. Refund or cancellation requests after March 1st will be reviewed and determined by the YSLC board and are subject to a $ 50.00 cancellation fee. Uniform orders CAN NOT be cancelled or refunded after January 31st.
