York Suburban Lacrosse Club :: Registration

The registration information on this page is applicable to youth teams (U9-U15) only. For registration information for the Men's High School, Women's High School and Women's Middle School teams, contact the York Suburban School District.

Registration for all 2009 teams is OPEN.

Follow the instructions below to register a player for the 2009 season.

  1. Download and complete the Registration Packet. The packet contains the following documents: a Registration Form, an Amateur Athletic Waiver & Release of Liability Form, a Medical Form, a Proof of Insurance Form and a Payment Form.     Please note: US Lacrosse membership is required to register with the club. You must register with US Lacrosse for the 2009 season. Return proof of payment (printed copy of your receipt) and your US Lacrosse membership number (provided to you following your US Lacrosse order) with your registration packet. You can obtain a US Lacrosse membership online using the link below. Visit the 'Membership Information' section and choose to 'Apply or Renew Online':

    Please note: A copy of the player's birth certificate needs to be attached to the registration packet. Please ensure a copy of the player's birth certificate is attached prior to submitting the completed registration packet.

  2. Return the complete registration packet with a check made payable to 'York Suburban Lacrosse Club' at the registration event, or mail to:

After you have submitted your registration packet, your packet will be processed and you will be contacted by the club to inform you everything is in good order. Until such time as your registration packet has been processed completely, participation in club activities where there is a possibility of injury, such as practices, scrimmages and games will be limited.
